What is wrong with me!? (I can't ejaculate normally!)

Hello. I’m a 22 year old male and I have been having noticeable problems now for a few months during and after masturbation. I do not know what is going on down there but something is definitely not working properly. I don’t know whether it’s prostatits, urethritis, some other inflammatory condition, a urinary tract or bladder infection, or if it’s something more serious like cancer (I really hope not).

Before I describe the symptoms, I should say that about two years ago I was put on anti-depressants for social anxiety and I took them on and off for a few months. When i was on them, I immediately developed anorgasmia (inability to ejaculate). it would literally take hours for me to ejaculate and there were times when I felt I possibly injured myself in the process, as I was always determined to finish no matter how long it took. Later on I decided to switch over to the natural herb anti-depressant St. John’s Wort, and although at the beginning of taking it everything was back to normal, the anorgasmia slowly crept back, just not as pronounced. The strange thing was that some days I could finish in 3 minutes, and on other days it took an hour or more, especially at night.

I have not been on any type of anti-depressant now for awhile, and I could ejaculate in little time like I did when I was a lot younger, but now another problem has developed. I’ve been very stressed out in recent months and I had been masturbating a lot more than normal. There were days when I would literally masturbate three times in the course of one hour. At this point right now, whenever I masturbate, what happens is that the automatic contractions “leading up” to ejaculation are extremely sharp, tight, and painful, as if the muscles down there in the perenium area are getting contracted to the point where it hurts. During ejaculation the contractions are equally sharp, tight, and painful. It’s like there’s some obstruction somewhere and ther muscles that transport semen out of the prostate are working extra hard tio move fluid.

After these “initial” symptoms, I feel a general discomfort in my groin area that lasts for a few days and a sensation like my bladder is full, even after urinating. It’s much harder to urinate as well; it doesn’t “flow” smoothly. Also, it feels swollen in the lower left part of my abdomen. The important thing though is that all these symptoms only seem to arise during and then after I masturbate, as if masturbating aggravates the condition. If I don’t masturbate, I don’t get any symptoms and everything feels normal. But ofcourse, my libido still exists and so it is almost like torture because I have the urge to masturbate but I know that if I do it’s going to aggravate the condition and the sympotms are going to reappear.

I know I should see a doctor, but what does this problem sound like? Do you think my past experiences with the anti-depressants and anorgasmia influenced my current problem, or is it a problem that’s been directly caused by over-masturbation in recent months? Please help!

OK>not a doctor myself, I'm about to give you the best advice anybody could right here. And you sound like an intelligent guy, so......you already know the answer. RUN, do not walk, to your phone and call the nearest urologist and make an appointment for as soon as they can see you. Find one that is "Board Certified" in urology. NOBODY else. NO exceptions. Tell them you have an urgent chronic problem and you are worried and need an appointment soon. Then make a copy of what you wrote here and give it to the doctor the minute you walk in the exam room. Tell him to read it and he will. I personally don't think it has anything to do with "over masturbation". Millions of people your age do this all the time and don't have these symptoms. Something else is going on. It may be something minor, BUT.....you MUST see the urologist and not wait. Waiting=bad. The exams are no big deal. So, get this looked at by a pro and they can help you I'm sure. I also don't think it has anything to do with the fact that you took some anti-depressants some time ago. That should not affect you now. But let the pro tell you this. Please>
do not wait. OK? I wish you luck but I'm sure you will be fine. It can be a lot of things, but diagnosis is the key to the whole thing.

Thanks for your words and support. I am definitely going to make an appointment with a urologist. Honestly though I've waited this long because I'm scared that I might have prostate cancer or something like thst. It's driving me nuts.

If you have had any type of prostate surgery this will happen. Actually 99% of men don't ejaculate after prostate surgery. The feeling of the climax is the same but nothing comes out of the penis. I had the HOLEP procedure so that is how I know.

OK>you are concerned that you might have something more serious going on, but.....let me just tell you that it would be very unusual for you at your age to have prostate cancer. So, don't think so much about that and concentrate on doing the right thing which is getting somebody to look at you and help you find out what the cause is. It can be many things, and I don't want to speculate here. Let an expert tell you.
Let them do a urinalysis and a blood test if they want to. Also, he will look for signs of infection, etc.
All these things can be treated. Many of your symptoms are very common and I'm sure they will come up with a diagnosis and help for you. Don't suffer with this any longer...you'll be glad you went this route.
