2 y/o "pee pee hurts"

My son had a UTI when he was 2 weeks old. They thought it was very odd for a boy to get. He was treated with antibx and has not had any problems since. That is up until about 2 months ago. He was acting like he was going to faint so we took him to the ER. They cath'd him and said he had ketones in his urine and that he must be dehydrated. During the cath, the nurse kept advancing the catheter after there was a urine coming out. He was screaming. I told her "that is enough. there is already urine." She did stop. Within an hour, he pee'd and when he did he cried and said his "pee pee hurt."
We went to our doc and she said that yes he was probably dehydrated and that is why he was acting fainty. I thought that was the end of it.
A week or so later, he again said his pee pee hurt. I looked, it looked ok. The next morning he was fine. This has happened a few times now and it got me thinking....I wonder if he does have some congenital thing and he wasn't dehydrated but rather his urine backing up or something and causing the ketones. The dehydration diagnosis never made sense as he was drinking all that day. I also wonder if he got some kind of damage when he got cath'd. She inserted it too far. Any thoughts???

I'm sure you have already seen a doctor about this issue and it may be resolved now but have you thought about having your son checked for kidney reflux. My daughter had the same symptoms when she was about 2 weeks old and it turned into a nightmare for me until I found out what was really going on and it almost cost her her life.

What timing. I posted this a couple months back. I did not end up seeing a urologist as he did not have a UTI and doc didn't think it was a big deal. Do you know that just today I had to call the doc again, after all these months, because he is STILL having pain. We are going to get a urine sample and go from there. Hopefully she will refer him to a urologist so I can get to the bottom of this.
He was worked up for reflux when he was a couple weeks old but all tests came back normal.
