resistant strainI am a pediatric nurse and the mother of 2 children with VUR. Both were diagnosed with hydronephrosis prenatally. My 2yo had one of the worst VCUG's the radiologist had ever seen w/ Gr 5 bilaterally. He was on amox and then Bactrim until he was 18mo and he then had a Deflux procedure, which thankfully has been very successful so far. He never had a UTI We will see how he does with potty training and having to hold urine in his bladder.
Now to my daughter...Postnatal renal US showed NO hydronephrosis despite Prenatal findings. VCUG x 1month showed Gr 4-5 bilaterally, and the ureters were not as tortuous as her brother's. She was on amox and then Bactrim begun at 3 months. She is now 6.5 months old and has been diagnosed with her first UTI She didn't have any real symptoms, just moderately strong urine the other morning and went to bed a little harder than normal the night before. No fever rectally, happy, normal appetite, active and interactive. We bagged her at home and her urine was clear pale yellow. We switched to amox right away and today we were notified that she has a resistant strain of Enterobacter cloacae, resistant to all oral abx except Cipro (a nasty little antibiotic, not recomended for children because of potential for cartillage damage, but benefits outweigh risks at this point). We are having renal labs drawn this week and will do a follow up urine with an office visit on Saturday. Does anyone have any experience with resistant bacteria this early on? I have to admit that I am in a bit of a panic and getting ahead of myself, seeing awful things in her future (colonization of resistant bacteria, urostomy, umbilical catheters, etc.)
Re: resistant strainmy daughter became resistent to the antiobiotics and had to be put on IV cipro in the hospital twice. Once when she had her first infection at 2.5 months old and once when she was 6 months old. I know this is awhile after your posting but make sure you give her probiotics mixed with formula or juice when she is on the therapy it will help with the loose bowels and stomach cramping. My daughter went through multiple drug resistance before she finally started slowing down with the infections at around 2.5 yrs old. Hope this helps.

Re: resistant strainMy daughter had her first UTI when she was 2.5 weeks old and she had to be hospitalized for 5 days with IV antibiotics. She was then put on profolactics and they didn't do crap. To sum it all up she has been in the ICU 8 times with UTI's with milder UTI's inbetween. She has been resistant to Amox, Bactrum, Cipro and a few others. She finally had to have surgery to fix the VUR which was grade 5. My daughter has been on Cipro alot and she hasn't had any problems with the meds. The only thing that they did when a resistant strain of infection came along was find out which antibiotic was sensitive to the infection and switch her to that. But I would be careful. My daughter never compained when she was little either until she had a really bad UTI. The only way I would know that she wasn't feeling well was by watching how much she ate and slept. When she wasn't feeling to hot she would eat less and sleep more. And she would be crankier than normal. I hope that this helps you out. And good luck with everything.
