Most urologists would prefer to treat this problem with drugs only. Unfortunately, this often is a temporary fix as the problem gets worse leading to BPH and possibly prostate cancer. Reaming of the gland through turp or greenlight laser does not necessarily rid one of the infection and inflammation which can be ongoing on a subliminal basis.
