Testicular Pain/ Scrotal Pain

I'm a 54 y/o MD with about 6 months of right epidymis pain and tenderness. It's at the head of the epididymis. This pain has occured off and on for about 15 years, though never so bad as now. The head of the epidymis is seemingly slightly enlarged and exquisitely tender. Urulogist's exam is negative. The pain does also correspond with twinges in my right side.
Sonogram reveals only a small right hydrocele CT without contrast is negative. Repeated courses of antibiotics have not been helpful. Urologist is hesitant to explore or perform epididymectomy because of the uncertainty of results, but after 6 months I'm afraid there's little else to do. Anyone with similar problems or advice? Thanks
