Testicular Pain/ Scrotal Pain

After looking around and reading other posts, I can't find anything exactly like what I've been experiencing since I was 5 years old. I'm now 32, have always been physically in shape and playing sports, but about 4 or 5 times a year, and ONLY after deficating, I will experience testicular pain that will take me to the floor in agony. It grows inside like when someone has kicked you in the groin and the pain will usually peak after about 3-4 minutes and then slowly subside for an average duration of 7-9 minutes. During these episodes, I can't talk, think or move and there is nothing I've been able to do to get them to shorten in duration. I feel nauseous but have never vomited during an episode and there is always the feeling of having to deficate again when the pain comes on, although nothing aggrivates the pain more than trying to go again. I recently started to shower with extremely hot water in order to help my testicles descend, as when these incidents occur, they will always pull up tight to my body in the scrotum. Sometimes this helps and others have no effect. No physician has been able to offer any suggestions at all and I've finally decided to research the internet for some help and clues to this problem.
Any thoughts or possible explanations would help tremendously. Thanks, cappy
