Testicular Pain/ Scrotal Pain

Hi all,

I am a 31 yo man with no medical problems

I am having light pain in my right testi, I have it for a while on and off. What can it be? what should I do, where should I go? I have another problem that bothers me and I am embarrassed to go to a doc, my scrotum sac is loose and hanging low and it is causing discomfort, especially when its hot. When penis is erected the sac shrinks and feels great. My penis is pretty small 6x5 when erect (is it small) and much smaller when not erect which is making it more difficult on my confidence and self esteem. Also, when i urinate i need to press betwwen the scrotum sac and anus to make sure everything goes out. any help with any of my issues is appreciated.
