Testicular Pain/ Scrotal Pain

My 4 year old has RSV which consists of some pretty severe coughing. Needless to say when we woke up this morning and he got off the toilet I noticed a good sized lump above his penis, it was his testes. I was alarmed, as I have 4 boys and never have seen anything quite like this. I took him to the ER later in the day for labored breathing and asked the ER doc about it; he did a very brief exam (they appeared normal at that time) and he said it could be all the coughing. My question is: is this normal? I noticed it again when we got home. Will they just go back to normal? Is this something I should be concerned with?
