Testicular Pain/ Scrotal Pain

My boyfriend (he is 48 years old) gets pain in the testicels when he tutch them and only when something tutch them i think i had it like this for many years and he very selldom go to the doctor becourse he is never seek.Its very high pain he gets if they gets tutch by anything
does anybody now what the problem can be ???
