Denied Insurance

I was denied Testim from my insurance company which I am finding is not uncommon.
I am hoping someone may know how to deal with them. I have had 2 blood tests in the last 90 days the 1st count was 297 the 2nd was 266 but their scale claims I am not low on Testosterone until I am below 245. I have been fighting this for 10 yrs and finally saw hope, only to be denied.
Thanks for any info you may have.
