Testosterone level of 48 and doc is not concerned??

hi
I have been seeing a urologist for several years. I have a low testosterone level and the best we can determine as the cause, is an injury to the testes when i was about 35. Anyway, my very latest level is 48, despite using 1 packet of AndroGel daily(5 gms).
I of course don't use it prior to the test(for that day only ) the doc , called the lab and wrote on the lab sheet I was not on androgel, used afterwords. thats misleading.I am on androGel, i just did not use it on the day of the test. I have asked for a repeat test, perhaps a lab error.
What could cause a level this low while taking androGel?
I'm 59 and otherwise good heath.

I could not see an edit function so i wanted to add that i do have depression, and the doc runs a psa b/c my father had prostate cancer. but at 59 I'm free of the usual suspect diseases, except asthma. I have had ED for several years and use cialis with some results.
