Relapsed UTI, Several Infections and Weight Loss

I am a 34 yr old female. I went to my OBGYN for follow up of bright red blood in urine pain while urinating, waking up at night to urinate, stomach spasms and moderate stomach and back pain. A urinalysis was performed and positive for infection. I was given an antibiotic shot in the office and a prescription for Fluconazole. Exactly 6 days later, the symptoms returned and were even worse. I was then having spasms in my lower back as well with extreme nausea and vomiting. I called the Dr.'s office and was told that my culture was a different strain that what they had treated me for and called me in a 7 day prescription of Nitro**? Anyhow, it is now two weeks later, I just went to the walk-in clinic (to avoid missing more work) and tested positive again for blood in the urine. Also, I have a fever, sinus infection, fluid on the ears and my throat is infected. I am having intermittent moderate pain in my back and stomach with intense spasms (some are waking me up at night). I have lost 11 pounds in 3 weeks. The Dr. at the walk-in clinic prescribed a 5 day dose of Levaquin. I followed up with my OBGYN via phone and they have referred me to a Urologist. The Urologist can't see me for two more weeks. At this point, I'm close to feeling like I should be admitted to the hospital but all of this is very expensive and I'm not wanting to miss more time from work. Considering my situation, would it be wise for me to wait the two weeks, find a Urologist that can see me sooner or go to the emergency room? I almost died due to a ruptured tubal and another time due to a ruptured appendix. I'm a tough cookie is what I'm trying to say. On the other hand, I have 3 kids and don't want to be stupid again by waiting too long to get taken care of. Any advice would be helpful. By the way, no other diagnostic tests were performed...only the urine sample that was sent for a culture.
